IKRON said an event affected its network and operations on or about December 23, 2025. In February 2026, it learned that a third party had accessed certain servers and files and extracted administrative or employment-related information associated with current and former employees. On or about March 4, 2026, IKRON learned that a second party had accessed an electronic health record environment and extracted client health and vocational-services information.\u003C\u002Fp>\u003Cp>The HHS Office for Civil Rights record lists 11,845 affected individuals and classifies the event as a Hacking\u002FIT Incident involving an electronic medical record and network server. This is a regulatory affected-person count, not an imported-record, file, patient, or employee count. The organization disclosed only February 2026 for awareness of the first access wave, not an exact day.\u003C\u002Fp>\u003Cp>Information varied according to each person's relationship with IKRON. Possible fields included a name, address, date of birth, Social Security number, and sometimes a driver's license number. Patient-service data may also have included a medical record number, health insurance information, diagnoses, medical history, intake and assessment records, treatment plans, clinical notes, appointments, and billing. Employment or vocational-services data may also have included participation and service records, intake information, notes or communications, education and work history, and, for some people, limited medication or general disability information associated with a service plan. No field should be assumed to have applied to every person.\u003C\u002Fp>\u003Cp>IKRON said it reported the incidents to law enforcement, isolated affected systems, disabled compromised accounts, reset passwords, and engaged external forensic specialists. It also announced 12 months of credit monitoring for individuals identified as affected. People should act according to the fields and terms in their own notices.
