According to the investigation, an unauthorized person accessed and copied certain files between August 14 and August 24, 2025. Expert MRI identified evidence of possible unauthorized access to its systems on September 2, 2025, began a forensic investigation, and reported the event to federal law enforcement.

What Information Was Affected?

The affected fields varied by person and may have included names, addresses, dates of birth, admission dates, diagnosis and treatment information, and driver's-license or state-identification numbers. Social Security numbers were also present for a subset of affected people.
