Liberty Solutions and Centra Health learned around March 17, 2026 that the file had been posted to the online developer platform and launched an investigation. The official notice says the file was connected to a Centra Health-managed project.

The file may have contained names together with addresses, dates of birth, physician names, dates and categories of service, contact information, and other demographic details. Social Security numbers were also present for some individuals.
